Creekside Transitional Care & Rehabilitation is a modern, state-of-the-art care center located in the heart of Meridian, Idaho. We are conveniently close to St. Luke’s Meridian Medical Center and just a short drive from major medical facilities throughout the Treasure Valley.
We feature 139 beds and beautiful spacious rooms in either private or semi-private settings. Guests will enjoy well-appointed accommodations, a diverse activities program, nutritious, satisfying menus, housekeeping and laundry services, and beautifully landscaped grounds.
